Client Consent & Privacy Agreement

1. Scope of Practice - Nationally Recognised Nutritionist (NRN)
📋 What is an NRN?
A Nationally Recognised Nutritionist (NRN) holds a Certificate IV in Nutrition (11046NAT) and is registered with the Nutrition Council Australia (NCA). NRNs provide nutrition education services within a defined Scope of Practice.
Services I CAN provide:
- ✅ Nutrition education for healthy individuals (NIL chronic conditions)
- ✅ General dietary advice and meal planning
- ✅ Healthy weight management for healthy clients
- ✅ Sports nutrition for recreational athletes
- ✅ Cultural food guidance (Vietnamese traditions)
- ✅ Digestive wellness education (non-clinical)
- ✅ Energy and vitality improvement advice
Services I CANNOT provide (require APD/GP referral):
- ❌ Medical nutrition therapy (MNT)
- ❌ Treatment or management of chronic diseases:
- Diabetes (Type 1, Type 2, Pre-diabetes, Gestational)
- Heart disease and cardiovascular conditions
- Kidney disease (CKD)
- Liver disease
- Cancer (current or recent)
- ❌ Eating disorder treatment or management
- ❌ Mental health conditions affecting eating behaviors
- ❌ Pregnancy or breastfeeding nutrition
- ❌ Infant and child nutrition (under 12 years)
- ❌ Diagnosis of medical conditions
- ❌ Prescription of therapeutic diets
🔄 Referral Process: If during our work together I identify that your needs fall outside my Scope of Practice, I am professionally obligated to refer you to:
- Accredited Practising Dietitian (APD) - for medical nutrition therapy
- General Practitioner (GP) - for medical assessment
- Psychologist - for eating disorders or mental health support
2. Consent to Nutrition Education Services
I, the undersigned client, hereby:
- Understand the NRN Scope of Practice as outlined above.
- Confirm that I have disclosed all relevant health conditions during pre-screening.
- Agree to inform Resonant Nourish immediately if my health status changes (new diagnosis, pregnancy, medications, etc.).
- Acknowledge that nutrition education services are NOT medical treatment or therapy.
- Accept that I may be referred to an APD or GP if my needs fall outside the NRN Scope.
- Understand that I remain responsible for my own health decisions and should consult my GP for medical advice.
3. Privacy & Confidentiality
Collection of Information
Resonant Nourish collects the following personal and health information:
- Name, contact details, date of birth
- Health screening information (conditions, medications, pregnancy status)
- Nutrition goals and dietary preferences
- Cultural and language preferences
- Emergency contact information
Use of Information
Your information will be used for:
- Providing nutrition education services
- Assessing suitability for NRN Scope of Practice
- Generating referral letters to APD/GP if required
- Appointment reminders and follow-up (with consent)
- Record-keeping for professional compliance
Storage & Security
- Client records are stored securely (electronic and/or physical)
- Access is limited to authorized personnel only
- Records are retained for 7 years as per professional requirements
- Data is protected with encryption and secure storage systems
Disclosure of Information
Your information will NOT be disclosed to third parties except:
- With your written consent
- For referrals to APD/GP/Psychologist (with your knowledge)
- If required by law
- In case of immediate risk to your safety or others
Your Privacy Rights
You have the right to:
- Access your personal information
- Request corrections to inaccurate information
- Withdraw consent for marketing communications (does not affect service provision)
- Request a copy of your client records
- Lodge a complaint if privacy is breached
4. Communication Consent
I consent to receive communications from Resonant Nourish via:
- ☐ Email
- ☐ SMS/Text message
- ☐ WhatsApp
- ☐ Phone call
Communications may include:
- Appointment reminders and confirmations
- Nutrition education resources and tips
- Updates about Resonant Nourish services
- Follow-up after consultations
Note: You can withdraw consent for marketing communications at any time without affecting your service provision.
5. Fees & Cancellation Policy
Service Fees
- Fees are communicated prior to booking
- Payment is required at time of service (unless other arrangements made)
- Receipts are provided for all transactions
Cancellation Policy
- Cancellations require 24 hours notice
- Late cancellations (less than 24 hours) may incur a fee
- No-shows may be charged full appointment fee
6. Professional Boundaries
Resonant Nourish operates under the following professional standards:
- Services are provided in a professional, respectful manner
- Professional boundaries are maintained at all times
- Evidence-based nutrition advice is provided
- No therapeutic claims are made about food products (TGA compliance)
- Cultural sensitivity and respect for Vietnamese traditions is prioritized
7. Complaints & Feedback
If you have concerns about the service provided:

- Formal complaint: Submit written complaint via email
- NCA complaint: Contact Nutrition Council Australia if unresolved
